How to Obtain Your Medical Records

Dear Patient,

Dr. Maria Rodriguez and Eileen Kerem, CFNP retired on December 31, 2021. We want to give you instructions on how to get your medical records. Dr. Rodriguez hired a Custodian of medical records company named CARIEND. They will keep your records for 10 yrs. The instructions below will instruct you on how to obtain your records. Due to HIPPA regulations only you can get the records for yourself or to have them sent to another provider. Therefore they will require some identification and sign a Release of Information Form which starts the process. Patients requesting their records need to complete a Records Release Authorization Form and provide a copy of government issued identification, as required by the HIPPA laws. The fastest way to process requests for copies of medical records is to visit Cariend’s website and submit your contact information at the link below – you will be sent an email with instructions to securely upload your authorization form and ID: https://www.cariend.com/request-records/

If you do not have access to a computer or internet, Cariend can be reached by phone at 404-991-7080. Please note the HIPPAA laws do not allow medical records to be released based on a phone or email request.
